For CA, FL, ID, IN, IL, TN, NC, NV, TX, UT, MO: A supervisor and the injured employee need to complete a report in Risk Wizard and call our workers’ compensation insurance carrier to report the claim immediately. Record the claim number on the Risk Wizard incident report.
