CERTIFICATE OF INSURANCE REQUIREMENTS

Items to verify on COI

• Property Entity Name & Horizon Entity Name with corporate address listed in the certificate holder box
• Description of Operations box specifies “certificate holders are named as additional insured (verbiage can very)
  • “ADDL INSD” BOX can also be checked off as proof certificate holders are covered under policy
• Minimum $2,000,000 in General Aggregate
• Minimum $1,000,000 in Bodily Injury and Property Damage Protection per occurrence
• If Workers Compensation policy is not on COI, request from vendor for proof they have current Worker’s Compensation Coverage

• Request a copy of the additional insured endorsement page(s) if not included with certificate
  • There’s a warning on the certificate that states you need an endorsement if the holder is an additional insured
